Description

Comprehensive Medication Management (CMM) is the standard of care that ensures each patient’s medications are individually assessed to determine that each medication is appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, and able to be taken by the patient as intended. 

This course is designed to cover the key foundational concepts of CMM through a knowledge-based section and then an application-based section. In the first part of the course, you will find presentations and resources that explore the topics of philosophy of practice, the patient care process, and practice management. In the second part of the course, participants will apply pharmacotherapy knowledge and obtain feedback through case studies.

Course Layout

The content for this course is broken into four knowledge-focused modules followed by two patient case studies. Each module contains a narrated video presentation by your instructor, and a short quiz. The case studies provide an opportunity to apply your knowledge and receive feedback. 

Objectives

Knowledge-based

• Define comprehensive medication management
• Explain the tenets of the philosophy of practice
• Discuss the significance of the patient’s medication experience and how this information is used to guide medication therapy decisions
• Describe the functions of the patient care process
• Describe the ten categories of medication therapy problems
• Explain the purpose of practice management

Application-based

• Assess patient-specific data and interpret these findings to identify medication therapy problems
• Establish goals of therapy for each medical condition and medication therapy problem identified
• Formulate recommendations for resolving medication therapy problems, providing rationale, justification, and evidence
• Create a plan for patient follow-up

Accreditation Statement

In support of improving patient care, University of Minnesota, Interprofessional Continuing Education is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.
	This application-based online educational offering and has been approved for 10 contact hours. The Universal Activity Number is JA0000810-0000-20-006-H04-P. Following successful completion of the online activity and an activity evaluation, participant data will be submitted to The Monitor within 10 days.

CE completion requirements

• Participants must complete the online activity, which includes passing four quizzes with a cumulative score of 70% or better and submitting two patient cases.
• Following successful completion of the above activity requirements as well as submission of a course evaluation and verification of completion form, participants will be able to access their CPE data directly from the Monitor. Once information is received by NABP, the tracking system will make CPE data for each participant available to the state boards of pharmacy where the participant is licensed or registered.
